How It Works

Eligibility Check

You’ll get a home visit from a nurse to verify your eligibility for the CDPAP program. You’ll also find out how many weekly hours of home care you can get.

Caregiver Designation & Enrollment

Once your CDPAP eligibility is confirmed, you’ll choose a caregiver. It can be one of your children, relatives, or friends. We’ll even come to your home to enroll you in the program.
